Adult Community Case Management
Adult Community Case Management Provides supports to adults with developmental disabilities from Fort Kent to Sherman. Case management duties and responsibilities include but are not limited to; home visits, making sure that individual’s needs are met, Representative Payee Services, monitoring medical/dental appointments, completing referrals for services/ programs, assistance in obtaining housing resources and completing paperwork for Maine Care Waiver Services, funding requests, and advocacy. CCM will update Service and Supports, critical information, and other required information and Assessments in the Enterprise Information System.
Youth Targeted Case Management
Youth Targeted Case Management serves children 0-20 years old with qualifying diagnoses such as mental health, autism, developmental disabilities, and chronic medical conditions. Using the wraparound process to assist children and families with identifying their strengths and using those to reach their identified goals in all areas of need. TCM’s are knowledgeable of the local resources and supports available to assist children and families in their journey through the lifespan.
