SUMMARY: This is a full time hourly position involving coordination, supervision, and administration of program services for adults with developmental disabilities.
DUTIES AND RESPONSIBILITIES include the following, other duties may be assigned.
Reviews and determines consumer eligibility for services.
Coordinates Representative Payee services
Coordinates medical, dental and other wellbeing appointments
Provides transportation for appointments if necessary
Problem solving with individuals to ensure daily life quality
Supporting daily living needs
Identify and acquire services that individuals qualify for
Complete assessments in Electronic Information System (EIS)
Record service notes in EIS
Coordinate meetings with providers of services
Travel to individual’s home, program or in the community
EDUCATION and/or EXPERIENCE: Bachelor’s Degree from an accredited four (4) year institution of higher learning with a specialization in psychology, mental health and human services, behavioral health, behavioral sciences, social work, human development, special education, counseling, rehabilitation, sociology, nursing or closely related field, or
Master’s Degree in social work, education, psychology, counseling, nursing or closely related field from an accredited graduate school.
CERTIFICATES, LICENSING, REGISTRATIONS:
Safety Care-within three months
Valid State of Maine driver’s license.
Enterprise Information System training.
Action Note training.
All Case Manager training required by DHHS.

WORK ENVIRONMENT: The noise level in the work environment varies from quiet to noisy. Travel within Aroostook County is required and on occasion, other parts of the state.
